Job summary

Heartshare Wellness LTD seeks a Social Worker to support youth in residential programs across Queens and Staten Island. You will provide individual counseling, crisis intervention, and trauma-informed care in collaboration with staff and external partners.

The role requires MSW, 2–4 years of relevant experience, and strong clinical judgment to implement evidence-based interventions and ensure safety, stability, and growth for youth in care.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Minimum 2–4 years of experience working with youth in a residential setting.
• Experience with evidence-based modalities (CBT, DBT, MI or equivalent).
• Ability to work across two sites and manage a caseload.

Responsibilities

• Provide individual counseling, crisis intervention, and ongoing support to youth.
• Utilize evidence-based modalities to address behavioral, emotional, and trauma-related challenges.
• Conduct psychosocial assessments, safety planning, and service planning.
• Facilitate therapeutic groups to promote social-emotional skills and engagement.
• Maintain presence across both sites to support daily program functioning.
• Partner with leadership and staff to maintain a trauma-informed home environment.
• Participate in team meetings, reviews, and staff psychoeducation.
• Coordinate referrals and collaborate with external providers to ensure continuity of care.
