Qualifications

• Texas LCDC Intern license or equivalent required.
• Experience in substance use disorder or behavioral health preferred.
• Bilingual English/Spanish preferred.
• Valid Texas driver's license and proof of insurance if driving for Santa Maria business.

Responsibilities

• Establish rapport with clients using motivational interviewing and engagement strategies.
• Provide weekly case management sessions including care coordination and follow-up.
• Develop and revise individualized service plans with client and team.
• Coordinate communication with referral sources and healthcare providers.
• Facilitate referrals to mental/physical health and social services; assist with transportation and benefits.
• Participate in treatment team meetings and staff reviews for progress updates.
• Collaborate with residential staff to ensure integrated care.
• Integrate family and community systems into service planning when appropriate.
• Maintain accurate, timely documentation in compliance with licensing and funding requirements (e.g., CMBHS).
• Ensure services are trauma-informed, culturally competent, and align with policies and funder expectations.
