Qualifications

• Master’s Degree in Social Work or Mental Health Counseling is required.
• New York State Social Work or Mental Health Counseling license is required.
• Two years post-graduate experience in medical, mental health or human services preferred.
• Excellent written and verbal communication skills.

Responsibilities

• Individual evaluation and psychosocial assessment using records and interviews.
• Provide social work emergency evaluations in the ED when applicable and discharge planning.
• Participate in interdisciplinary treatment planning and crisis intervention.
• Document services timely and thoroughly for all interventions and planning.
• Coordinate post-discharge resources and aftercare for recovery.
• Collaborate with families, outpatient providers, and community supports.
