Job summary

Heartland Family Service in Iowa is seeking a qualified Nursing professional to join the Assertive Community Treatment Team. The role focuses on psychiatric assessment, medication monitoring, and coordinating treatment with the team.

You will act as a case manager for clients with chronic mental illness, ensure integrated care, and support reintegration to the community. Compensation up to $83,000 with a Monday–Friday schedule.

Qualifications

• Bachelor’s or higher in Nursing preferred.
• Nursing license required – RN preferred.
• Two years of experience in substance abuse or mental health setting.

Responsibilities

• Plan, implement and evaluate programs of care for chronically mentally ill clients.
• Act as case manager for clients on own caseload and others on the team.
• Coordinate care with the ACT psychiatrist/nurse practitioner and other agencies.
• Provide medication management and clinical record maintenance.
• Support in inpatient transitions and community reintegration.
