Job summary

Heartland Family Service in rural southwest Iowa seeks a part-time Psychiatric Nurse to work on the Rural ACT team across nine counties. The role is on-site with remote/hybrid elements and daily travel; 10–20 hours weekly.

The nurse will assess and monitor psychiatric, medical, and social needs, oversee medication monitoring with the team psychiatrist/nurse practitioner, and coordinate treatment with providers.

Qualifications

• RN license required or eligible.
• Experience in behavioral health/substance use settings preferred.
• Working knowledge of primary health care issues.

Responsibilities

• Plan, implement and evaluate care and therapeutic interventions for chronically mentally ill clients.
• Act as case manager for clients on caseload and collaborate with team.
• Ensure coordinated, best-practice care and multi-agency coordination.
