Qualifications

• Bachelor’s Degree in Human Service field.
• Three years in case management, vocational or skill development experience.
• Excellent communication and interpersonal skills.

Responsibilities

• Client assessment of individual skills/interests and plan for employment/education goals.
• Assist clients in developing skills in daily living, social and interpersonal relationships, and occupational capabilities.
• Provide support and consultation to team members and community partners.
• Maintain all contact documentation and comply with contract conditions.
• Provide problem solving and crisis intervention as needed.
• Coordinate services with other community agencies and resources.
• Provide service by helping clients manage their finances.
• Display a courteous and caring attitude to clientele, volunteers, and visitors.
• Cooperate and collaborate with program area staff, volunteers, and other agency staff.
• Be dependable and punctual regarding scheduling and attendance.
• Abide by all program and agency procedures, policies, and requirements.
• Develop personal and program-related skills through internal/external training opportunities.
• Perform other program-related duties as assigned.
• Create, maintain and share a dynamic self-care plan.
• Strive to make connections between the agency and the larger community for fundraising and friend-raising.
• Essential functions performed on company site or designated workspace.
