Job summary

Tri-County Behavioral Healthcare is seeking an Adult Probation Intensive Case Manager to provide continuity of care, case management and psychosocial rehabilitative training for adults on felony probation. The role involves community-based services across the designated area, trauma-informed practice, and travel to client homes.

Requirements include a Bachelor's in Human Services, QMHP qualification, and 3 years of relevant experience; a valid driver’s license and background checks are required.

Qualifications

• Bachelor's Degree in the Human Services Field.
• QMHP required or eligible.
• 3 years' experience with persons with mental health challenges.
• Clear criminal record validated by checks prior to and annually.

Responsibilities

• Links adults to community resources and supports probation program requirements.
• Performs assessment and treatment planning activities.
• Provides services in accordance with contract requirements.
• Works as part of a community mental health team serving clients in home and community settings.
