Job summary

Highland Rivers Behavioral Health is seeking a Case Manager for Intensive Case Management to support adults with serious mental illness through an individualized recovery plan. The role involves coordinating services, visiting homes, transporting clients, and advocating for resources to help maintain community-based living.

You will develop an IRP with medical, behavioral, social, educational, housing, and financial needs, work with CORE providers, document carefully, and meet weekly

Qualifications

• Community-based position.
• Must visit individual's homes.
• Assist individuals with service needs and IRP development.
• Partner with CORE services provider in the IRP.
• Maintain confidentiality per HIPAA policies.
• Meet weekly contact and productivity standards.

Responsibilities

• Coordinate services to develop and implement the IRP.
• Visit clients in their homes and provide transportation.
• Advocate and link with other agencies to secure resources.
• Document activities and ensure eligibility criteria are met.
• Attend treatment team meetings as directed by Program Supervisor.
