Cummins Behavioral Health Systems, Inc. is seeking an Integrated Health Specialist in Avon, Indiana.

Qualified candidates should have a background in social work, human services, and/or mental health to serve youth through adults at our Hendricks County outpatient office. The role delivers trauma-informed, recovery-oriented services across home-based, community, office, and virtual settings, coordinating care, supporting treatment planning, crisis prevention, and long-term recovery outcomes.

Qualifications

• Bachelor's degree in Social Work, Mental Health Counseling, Psychology, Sociology, or related human services field is required or
• Associate's degree in human services or related field plus 4 years direct care experience
• High school diploma or GED with four years direct care experience in social services, behavioral health, school-based, etc.
• Two years' post-bachelors work experience in mental health is preferred.

Responsibilities

• Deliver psychosocial skill-building and behavioral management strategies to support emotional regulation and daily living skills.
• Utilize stagewise case management to support participation in services.
• Develop and maintain person- and family-centered treatment plans in collaboration with internal teams and external providers.
• Connect individuals and families to appropriate services across physical health, behavioral health, substance use, housing, and financial assistance.
• Identify and respond to safety risks by developing and implementing crisis and relapse prevention plans.
• Engage in ongoing professional development through continuing education, in-service training, and supervision.
• Perform other related duties as assigned.
