Qualifications

• Bachelor's degree in Social Work, Mental Health Counseling, Psychology, Sociology, or related field is required.
• Associate's degree in human services or related field with 4 years direct care experience.
• High school graduate or GED with four years direct care experience.
• Two years' post-bachelors work experience in mental health preferred.

Responsibilities

• Deliver psycho-social skill-building and behavioral management strategies to support emotional regulation, communication, problem-solving, and daily living skills across settings.
• Utilize stagewise case management to support participation in services.
• Develop and maintain person- and family-centered treatment plans with internal teams and external providers.
• Connect individuals and families to appropriate services across physical health, behavioral health, substance use, housing, and financial assistance.
• Identify and respond to safety risks by developing crisis and relapse prevention plans.
• Engage in ongoing professional development through continuing education, in-service training, and supervision.
• Perform other related duties as assigned.
