Qualifications

Responsibilities

• Deliver psychosocial skill-building and support emotional regulation, communication, problem-solving and daily living skills across home, community, office, and telehealth settings.
• Utilize stagewise case management to support individuals and families in starting and maintaining services.
• Develop and maintain person- and family-centered treatment plans with internal teams and external providers.
• Connect individuals and families to services across physical health, behavioral health, housing, and financial assistance.
• Identify and respond to safety risks by creating crisis and relapse prevention plans.
• Engage in ongoing professional development and supervision.
• Work with individuals residing in a semi-independent living facility.
• Perform other related duties as assigned.
