Job summary

Health Connect America, Inc. seeks a Family Centered Treatment Specialist to deliver FCT services for children, adolescents, and families, primarily in home and community settings, with a focus on trauma-informed care and family engagement.

You will collaborate with a Treatment Team to develop and revise Service Plans, provide psychoeducation, and ensure timely documentation and coordination with medical and non-medical providers. A strong commitment to client-centered care is essential.

Qualifications

• Minimum of a Bachelor's Degree in human services or related field.
• Three years’ experience working with youth or families.

Responsibilities

• Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
• FCT Certification is required and must be completed within one year of employment to continue to provide FCT services.
• Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
• Provide 24/7 on-call coverage to a caseload of families and children, with a flexible work schedule.
• Develop Service Plan in coordination with Treatment Team and maintain the plan per designated authorization processes.
• Conduct ongoing revisions of Service Plan and be responsible for implementation of Service Plan.
• Provide psychoeducation as indicated in the Service Plan.
• Coordinate and oversee the initial and ongoing assessment activities.
• Convene the Treatment Team for Service Plan and consult with medical and non-medical providers to ensure integrated care.
• Ensure linkage to clinically appropriate services including evaluations.
