Job summary

Mental Health Association in Orange County is seeking a part-time Home to Stay (HTS) program assistant to support residential services for 12 individuals, providing trauma-informed, person-centered care on-site and in the community.

You will maintain charts in Footholds, coordinate with landlords, assist with daily living skills, and respond to crises per agency policy; CPR/First Aid certification required.

Qualifications

• High School Diploma required.
• 1 year experience with individuals with mental illness or serious emotional disturbance.
• Ability to work independently with minimal supervision.
• Effective written and oral communication skills.
• Must be organized, self-motivated and able to coordinate multiple tasks.
• Must maintain confidentiality and comply with OMH background checks.

Responsibilities

• Establish and maintain rapport with all program participants.
• Maintain positive external relationships with providers, family members, and others involved in treatment.
• Take clients on social outings and scheduled trips to parks, shops, and grocery stores every weekend.
• Instruct, assist and supervise daily living skills (house maintenance, medication disbursement, finances, hygiene, health care, social/recreational activities, laundry, groceries).
• Maintain participant charts and data entry into Footholds.
• Encourage responsible decision making and contribute to individual service plans.
• Intervene in crisis situations per agency policies and report objectively.
• Protect confidentiality and participant rights.
• Maintain CPR/First Aid certification.
• Develop relationships with landlords on behalf of clients.
