988 Lifeline in New York is seeking a part-time residential support staff for the Home to Stay program. You will assist 12 individuals with intensive daily supports on site and in the community, providing trauma-informed care using a strength-based, person-centered approach.

Responsibilities include crisis intervention, documenting in Footholds EMR, coordinating with landlords, and promoting rehabilitation through social and recreational activities. Requires HS diploma and NYS driver’s license.

Qualifications

• High School Diploma required.
• 1 year experience working with individuals with mental illness or emotionally disturbed.
• Valid NYS Driver’s License and clean driving record for agency coverage.

Responsabilités

• Establish rapport with program participants.
• Maintain external relationships with providers, families and others involved in treatment.
• Assist with daily living activities and house maintenance tasks.
• Monitor medication disbursement and self-medication.
• Document in Footholds EMR and maintain participant charts.
• Intervene in crisis situations per agency policy and report objectively.
• Ensure participant confidentiality and rights are upheld.
• Facilitate social and recreational activities to aid rehabilitation.
• Attend training sessions for skill development.
• Assist in planning and implementing individual service plans.
