Job summary

MHA (Mental Health Association) in Massachusetts seeks a Case Manager to provide outreach, assessments, and engagement for participants in the BSAS Low-Threshold Housing Program. Focus on trauma-informed, person-centered support to help secure and maintain housing while addressing related needs.

Role includes collaboration with a multidisciplinary team, development of IAPs, and crisis management. Pay is $19-21 per hour; standard hours are Mon-Fri, 9am-5pm, with some flexibility.

Qualifications

• Bachelors in human services or related field required.
• Requires 6 months experience with mental health, substance use, or housing instability.
• Must be at least 21 years old.
• CPR and First Aid certifications must be current.
• Valid driver's license, reliable vehicle, and adequate insurance.

Responsibilities

• Coordinate and complete program entry documentation (eligibility screenings, assessments, treatment plans).
• Collaborate with a multidisciplinary team to develop and implement Individualized Action Plans (IAPs).
• Develop, implement, monitor individualized support strategies and crisis plans.
• Assist participants in securing and maintaining housing, including landlord outreach and tenancy education.
• Provide tenancy support, including moves, utilities setup, address changes and housing standards.
• Support participants in employment, education, benefits, and community resources for stability.
• Promote community integration, daily living skills, transportation, budgeting and self-advocacy.
• Provide emotional support and harm-reduction counseling, coordinate health appointments, respond to crises.
