Qualifications

• Experience delivering trauma-informed, recovery-oriented services is preferred.
• Ability to develop person-centered treatment plans with internal and external teams.

Responsibilities

• Deliver psycho-social skill-building to support emotional regulation and daily living skills.
• Utilize stagewise case management to support service participation.
• Develop and maintain person-centered treatment plans.
• Connect clients to services across physical health, behavioral health, housing and finances.
• Identify safety risks and implement crisis and relapse prevention plans.
• Engage in ongoing professional development and supervision.
• Perform other related duties as assigned.
