Job summary

Center for Family Life and Recovery, Inc. is seeking a Family Stabilization & Integrated Care Navigator to provide intensive, family-centered case management for TANF-eligible households facing housing instability, mental health challenges, and other barriers to self-sufficiency.

This role coordinates services across multiple systems to promote stability, resilience, and family well‑being. You will assess needs, develop stabilization plans, advocate for families, and connect them with housing,

Qualifications

• Bachelor's degree in social work, Human Services, Psychology, Counseling, or a related field required.
• Master's Degree preferred.
• Strong documentation, organizational, and communication skills.
• Ability to work independently while maintaining effective communication with supervisors and team members.

Responsibilities

• Conduct comprehensive assessments to identify family strengths, needs, and barriers related to housing, employment, mental health, substance use, developmental disabilities, domestic violence, and overall family functioning.
• Develop individualized family stabilization plans with measurable goals to support self-sufficiency and long-term well-being.
• Provide intensive family-centered case management and ongoing care coordination services.
• Assist participants with attending appointments, meetings, activities, and service requirements necessary to achieve identified goals.
• Provide crisis intervention and same-day support services, including connection to emergency housing, behavioral health, and substance use treatment resources.
• Maintain confidentiality and comply with all regulations; ensure documentation is accurate and timely.
