Job summary

Eagle Quest seeks a dedicated Family Resource Specialist to serve youth and families in the community. You will conduct assessments, develop individualized case plans, coordinate services, and advocate within schools and community systems using trauma-informed, strengths-based approaches.

The role includes home visits, office meetings, and travel to connect families with needed resources while documenting progress and collaborating with multidisciplinary teams.

Qualifications

• Bachelor's degree in social work or related field.
• Valid driver's license and reliable transportation.
• Ability to travel and conduct home visits.
• Strong written communication and organizational skills.
• Experience with youth and families preferred.

Responsibilities

• Conduct family needs assessments.
• Develop individualized case plans and safety plans.
• Monitor progress and adjust service plans as needed.
• Provide one-on-one support and intervention services.
• Home visits and community-based meetings when appropriate.
• Coordinate resources and advocate within schools and community systems.
• Maintain timely, accurate documentation and collaborate with teams.
