Job summary

The Family Support Center at Canopy Village is seeking a Family Case Manager to engage families, assess needs, and coordinate services through Canopy Village. This role emphasizes trauma-informed care and collaboration with partner providers to support youth and families in navigating resources and sustaining stable living situations.

You will create client-centered plans, coordinate with treatment teams, and facilitate groups and workshops for youth and parents, contributing to prevention

Qualifications

• Assess family and youth needs using validated screening tools and refer to internal/external services.
• Create client-centered case plans.
• Coordinate services for families, assist navigating resources, and support stable living environments.
• Provide after-care follow-up and proactive resource mining for at-risk youth.
• Collaborate with external providers, community partners, and Canopy Village teams.
• Facilitate groups and workshops for youth and parents.
