Job summary

VisionQuest (VQ) is seeking a Case Manager for Family Preservation and Prevention Services to provide ongoing, family-centered case management for children, youth and caregivers.

The role involves assessing family needs, developing service plans, coordinating referrals, and ensuring compliant documentation across state requirements. Travel within the service area, flexible hours, and collaboration with schools and providers are expected.

Qualifications

• Bachelor’s or Associate’s degree in the behavioral sciences, human services or social services fields preferred.
• 1 year case management or child welfare experience preferred.
• Must be 21 years of age or older.
• Flexible schedule (evenings, weekends, holidays) to support program services.
• Cleared background checks from appropriate entities (including Child Abuse and FBI).
• Must have a valid driver’s license to facilitate program services as required.

Responsibilities

• Engage children, youth, and caregivers using trauma-informed, strengths-based, and family-centered practices.
• Complete ongoing assessments of family needs, strengths, safety concerns, protective factors, and barriers to service participation.
• Develop, implement, and monitor individualized service plans in collaboration with families and involved agencies.
• Conduct required monthly in-home visits with all children and maintain additional contact based on family need and agency direction.
• Monitor child safety, family stability, service participation, and progress toward goals; report emerging concerns as required.
• Coordinate access to behavioral health, medical, educational, housing, and other supports for families.
• Provide practical support, coaching, and resource navigation to strengthen caregiver capacity.
