Job summary

VisionQuest (VQ) is seeking a Case Manager to support youth and families by linking them to vital community resources and helping them engage in prosocial activities. You will conduct in-home visits, assess needs, and facilitate school enrollment.

This role is crucial in empowering families and teaching life skills to adolescents. Strong interpersonal communication and a Bachelor's degree in a relevant field are required, along with the ability to work with a diverse population.

Qualifications

• Must be 21 years of age or older.
• Bilingual in English and Spanish is required.
• Case management experience is strongly encouraged.
• A minimum of one year of related work experience is preferred.

Responsibilities

• Engage with youth and families non-judgmentally.
• Schedule and conduct in-home visits.
• Conduct comprehensive intake assessments.
• Provide tailored community resources.
• Advocate for youth across various settings.
