Health Connect America is seeking an Integrated Care Coordinator in North Carolina to serve as a key connector for clients, providers, and community resources. You will oversee care planning, coordinate services, and monitor progress to promote comprehensive behavioral and physical health outcomes.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104. One of the following:
• Experience must be full‑time and supervised, serving children and adolescents with a primary mental health diagnosis.
• For care managers serving LTSS needs: Two years of prior LTSS/HCBS coordination and care delivery monitoring.

Responsibilities

• Oversee care planning, coordination, and monitoring for assigned clients across behavioral and physical health goals.
• Coordinate and monitor services across treatment teams, primary care providers, and community resources.
• Facilitate follow‑up services after hospitalizations or transitions to support smooth care.
• Monitor progress, track gaps, and verify enrollment and payer status to ensure access to services.
• Provide health education, guidance, and crisis response support to clients and families.
• Maintain timely documentation of care coordination in line with standards.
• Support clinic operations including scheduling, reminders, vitals, and charting with nurses for integrated care clients.
• Participate in marketing, outreach to referrals, and education of prospective clients.

Bachelor's in human services field

Bachelor's in non‑human services field

NC license or board‑issued certification in a human services profession
