Job summary

Nexus Family Healing is seeking a part-time Parent Partner for our Youth ACT team in Mendota Heights, MN. You will provide trauma-informed, family-guided peer support to parents of youth served by the program, collaborating with families and providers to reduce hospitalizations and support community living.

Qualifications

• High School diploma or equivalent, 21+ years of age.
• Experience as primary parent/caregiver of a child with emotional, behavioral, or mental health challenges.
• Experience with children’s mental health systems (DHS, DOC, social services, probation, or mental health).
• Successful background check completion.
• Willingness to complete DHS-approved CFPS training and exam.
• Valid driver’s license and ability to meet driving requirements.
• Open to meeting cultural needs of diverse populations.

Responsibilities

• Increase family involvement in treatment using trauma-informed, strength-based engagement techniques.
• Build community relationships to enhance resources and referrals.
• Engage families entering the program and assist in treatment goal development.
• Advocate for families during and after treatment with other providers.
• Provide parent skill-building education and lead family support groups.
• Respond to family crises promptly and collaboratively.
• Document work with families in the electronic health record.
• Link youth and families to appropriate community services and follow up.
