Job summary

JEVS Human Services is seeking a START Coordinator to lead community-based assessments, crisis prevention, planning, and therapeutic coordination for individuals with IDD and co-occurring mental health needs. You will stabilize clients in crisis, collaborate with families and providers, and coordinate trauma-informed, person-centered planning within an integrated service model.

Qualifications

• Master’s degree in social work, counseling, psychology, or related Human Services field.
• Must pass all required background checks.
• Must become START Coordinator within 12 months (training provided).

Responsibilities

• Manage a caseload of 20-30 individuals with dual diagnoses (IDD and behavioral/mental health needs).
• Develop Cross-Systems Crisis Prevention and Intervention Plans (CSCPIPs) using strengths-based, trauma-informed approaches.
• Conduct assessments and interventions with the clinical team and natural support systems.
• Coordinate additional therapeutic supports such as coaching and respite services.
• Serve as a clinical liaison across systems (behavioral health, developmental services, medical, educational, and emergency services).
• Maintain relationships with families, caregivers, and external providers to promote integrated care planning.
• Facilitate interdisciplinary support meetings to ensure coordinated service delivery.
• Share in 24/7 on-call rotation for crisis response (telephonic within 1 hour, in-person within 2 hours).
• Document in SIRS and EMR systems; track outcomes and support quality improvement.
• Participate in trainings, CEUs, and clinical conferences; obtain START Coordinator certification and CPR/First Aid.
