Job summary

Qualifications

• HS diploma and at least 1 year of related experience.

Responsibilities

• Provide real time, in-person and/or phone emergency crisis intervention support for youth 24/7.
• Attend and actively participate in treatment team meetings and other youth service plan related meetings.
• Enter content such as face-to-face contacts, ERC notes, and Behavior Support Plans into CNNX.
• Work with Aftercare Coordinator and Case Planning team to maintain a safe transition plan for youth in Aftercare.
• Support youth and families in de-escalation and trauma-informed care across placement and community settings.
