Job summary

State of Connecticut, Department of Mental Health and Addiction Services, seeks a Clinical Social Worker for the Outpatient Team in Torrington, CT. Qualifications

• Licensure as a CT clinical social worker
• Experience in outpatient behavioral health settings
• Ability to work within a multidisciplinary team
• Trauma-informed care experience

Responsibilities

• Provide individual, group and family therapy within an outpatient team
• Develop psychosocial assessments and recovery plans
• Coordinate care with contract agencies and community providers
• Monitor treatment progress and document in health records
• Participate in case conferences, trainings, and program committees
