Job summary

Qualifications

• LMSW or LCSW licensure required.
• At least 2 years of relevant experience with diverse populations.
• Bilingual English/Spanish, English/Cantonese or English/Mandarin a plus.
• Experience with trauma-informed care and SIFI a plus.

Responsibilities

• Provide trauma-informed, healing-centered assessments, therapy, counseling and advocacy for a caseload of at-risk children and families.
• Deliver services at school, home, after-school programs, or community sites.
• Develop individualized treatment plans and collaborate with partners.
• Maintain complete case management records and EHR documentation per Medicaid rules.
• Participate in meetings, supervision, and community workshops.
• Travel to families' homes, schools and communities in Manhattan and Brooklyn.
