Job summary

JEVS Human Services in Philadelphia, PA, invites applications for a START Coordinator to lead community-based assessment, crisis prevention, planning, and therapeutic coordination for individuals with IDD and co-occurring mental health needs.

You will provide 24/7 on-call crisis response, coordinate cross-system services, document in SIRS/EMR, and pursue ongoing professional growth within a supportive clinical team using trauma-informed care and START protocols.

Qualifications

• Master’s degree in social work, counseling, psychology, or related Human Services field.
• Certified START Coordinator within 12 months (training provided).
• Valid PA driver’s license and ability to travel within the community.
• Must pass all required background checks.

Responsibilities

• Manage a caseload of 20-30 individuals with IDD and co-occurring needs.
• Develop CSCPIPs using strengths-based, trauma-informed approaches.
• Conduct assessments and coordinate services with the clinical team and natural supports.
• Coordinate additional therapeutic supports such as coaching and respite services.
• Lead cross-system collaboration and crisis planning meetings.
