Job summary

WellPower is seeking a Clinical Case Manager I/II to provide comprehensive behavioral health case management within a continuum of services. The role includes assessments, care planning, and documentation to support recovery and community integration.

This field-based position requires regular jail, court, hospital visits and reliable transportation; a valid Colorado driver’s license and vehicle with business-use coverage are required. Competitive salary range and benefits are offered.

Qualifications

• Knowledge of trauma-informed principles and practices.
• Knowledge of methods of psychotherapy.
• Knowledge of community resources.
• Ability to assess crisis situations and intervene appropriately.
• Familiarity with the DSM-V and diagnostic techniques.
• Effective written and verbal communication skills.
• Ability to work flexible hours.
• Ability to maintain confidentiality and prepare written materials.

Responsibilities

• Divert individuals likely to be incompetent to proceed from the criminal justice system into care coordination.
• Attend court with clients (Competency Docket & Outreach Court) and coordinate transportation.
• Provide transportation to clients in your vehicle; mileage reimbursed with business-use insurance.
• Collaborate with a multidisciplinary team including legal, clinical, and housing partners.
• Engage in weekly outreach in jails, shelters, hospitals, and the community.
• Assist clients with accessing basic needs, benefits, employment referrals, and housing placement.

Education

Bachelor (CM I) or Master (CM II) Degree in psychology, social work, or related human services degree
