Job summary

WellPower in Denver, CO is seeking a Clinical Case Manager to provide comprehensive psychosocial and behavioral case management within a continuum of high to low intensity services, helping individuals meet basic needs and work toward recovery. You will document consumer interactions and coordinate transportation in your vehicle (insurance costs reimbursed).

Qualifications

• Bachelor (CM I) or Master (CM II) degree in psychology, social work, or related human services field.
• One year experience in mental health or human services preferred.
• Valid Colorado driver’s license and a recent clean driving record; ability to transport clients.
• Ability to obtain and carry Business Use coverage on auto policy (reimbursed).

Responsibilities

• Help people served meet basic needs for food, clothing, shelter, safety, and access benefits.
• Coordinate transportation for people served, using personal vehicle when needed.
• Provide psychosocial services to improve or maintain functioning and daily living skills.
• Support clients in finding employment, housing, and accessing community resources.
• Engage families and community partners to maximize benefits and program engagement.
• Maintain trauma-informed environment and complete clinical documentation.
