Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Counseling or related field.
• Experience with transition-aged youth, foster youth, housing programs or community-based services preferred.

Responsibilities

• Provide direct trauma-informed support to transition-aged youth and young adults with histories of foster care and housing instability.
• Develop individualized service plans and assist participants in achieving housing stability and self-sufficiency.
• Coordinate access to housing resources, transportation, education, employment, healthcare and behavioral health services.
• Utilize strengths-based and motivational approaches to empower participants to navigate barriers and build resilience.
