Job summary

The City of Lakewood seeks a part-time PREVAIL Case Manager to support youth and adults involved with the justice system by linking them to services and resources. You will work with the Lakewood Police Department and community partners to reduce recidivism and improve quality of life.

Responsibilities include direct client support, trauma-informed assessments, goal planning, and referrals. Strong documentation and teamwork are essential.

Qualifications

• Bachelor's degree in Psychology, Social Work, Human Services, Criminal Justice or related field.
• Two or more years of experience with diverse populations facing substance use, mental health, or housing insecurities preferred.
• Ability to work collaboratively with law enforcement and community service partners.

Responsibilities

• Engages program participants across varied settings to connect them with services.
• Provides direct support to clients enrolled in PREVAIL.
• Applies trauma-informed care and motivational interviewing to identify goals.
• Builds rapport and maintains professional relationships with individuals needing support.
• Participates in supervision and uses a problem-solving approach.
• Maintains inventory of community resources and makes referrals as appropriate.
• Represents PREVAIL team professionally to the public and partners.
• Operates department software including Word, Excel, PowerPoint, Outlook, and CAD/RMS.
