Job summary

Community Solutions Inc. is seeking a Full-Time, On-Site Case Manager for our Bridgeport AIC program. The role involves intake, assessment, case record management, and discharge preparation within a trauma-informed framework.

As part of the AIC program, you will facilitate general case management duties, participate in team meetings, and ensure confidentiality and timely reporting to supervisors. Starting wage is $25.72 per hour with full-time, on-site scheduling.

Qualifications

• Bachelor’s degree and one year experience in case management or similar field. Education may be substituted with experience on a year for year basis.
• Excellent oral, written and interpersonal skills.
• Ability to engage and develop collaborations with community agencies.
• Previous assessment experience preferred
• Experience with Microsoft Office.

Responsibilities

• Within 5 days of referral collect all intake information and enter into CSI and CSSD data collection systems on the day the data is obtained.
• Perform client needs assessments within 5 days of intake and enter data into CSI and CSSD data collection systems on the day the data is obtained.
• Initiate clients into group within 15 business days from the date of referral.
• Develop and document individual service plan goals with clients within 5 days of intake and review with client on a bi-weekly basis.
• Schedule, conduct, and document individual counseling sessions and enter each session into CSI and CSSD data collection system on the day the session is held.
