Job summary

Advanced Behavioral Health, Inc. is seeking an Intensive Care Manager (ICM) to support clients with severe mental illness transitioning from nursing home care back to the community.

The hybrid role involves home-based work with frequent travel across southwestern CT, including the Bridgeport–Stamford area, four days per week. You will lead clinical assessments, coordinate discharge plans, and develop recovery plans while ensuring compliance with program guidelines.

Qualifications

• Master's degree in a related field is required.
• Licensure as LCSW, LPC or LMFT, or eligibility is required with company assisting with clinical hours.
• Minimum of 3 years of clinical experience in behavioral health.

Responsibilities

• Travel statewide to assess and evaluate potential clients residing in nursing homes.
• Coordinate discharge plans and contingency recovery services for clients.
• Develop and review individual recovery plans and service authorizations.
• Collaborate with providers and teams to ensure coordinated care and transitions back to the community.
• Maintain knowledge of behavioral health benefits and coverage options.
• Prepare and file required paperwork and care coordination records.
• Provide client and family education and linkage to community resources.
• Participate in program development and quality improvement activities.
