Job summary

Canvas Health is seeking a Case Manager, MCHN to coordinate health and behavioral health services for adults in Washington and Anoka County.

You will work with a multidisciplinary team in a trauma-informed, person-centered approach to ensure referrals are completed and care is monitored over time.

Qualifications

• Bachelor's degree in social work or human services field.
• Two years' experience with mentally ill or chemically dependent individuals, including community-based mental health or chemical health work with adults.
• Experience working with diverse populations.
• Knowledge of local social and human service networks.
• Experience with Electronic Health Record systems.

Responsibilities

• Coordinate care across the treatment team, ensuring referrals are completed and services monitored over time.
• Provide therapeutic skills interventions to adults to support engagement in community-based mental and chemical health care.
• Collaborate with providers to address physical, mental health, and overall well-being needs of clients.
