Canvas Health is seeking a MCHN Case Manager to work with a multidisciplinary treatment team to address the physical health, mental health, and overall well-being needs of adults in the community.

The role emphasizes trauma-informed, person-centered care, coordination of referrals, monitoring services, and delivering therapeutic skills interventions; requires a bachelor’s degree in social work or human services and experience with EHR systems.

Qualifications

• Bachelor's degree in social work or in the human services field.
• Two years' experience preferred working with mentally ill and/or chemically dependent individuals.
• Experience with diverse populations.
• Knowledge of local social and human service networks.
• Experience with Electronic Health Record systems.

Responsibilities

• Coordinate care across physical health, mental health, and chemical health services.
• Ensure referrals are completed and services are monitored over time.
• Provide therapeutic skills interventions to adults to engage in community-based care.
