WHITE MOUNTAIN APACHE BEHAVIORAL HEALTH SERVICES, INC. is seeking a Thrive Case Manager to provide supportive behavioral health services to diverse client groups. You will coordinate services across clinical teams and community resources, arranging transportation and advocating for clients with service providers.

Responsibilities include maintaining intake data, developing treatment plans, and participating in weekly clinical staffings to ensure timely, coordinated care.

Qualifications

• High school diploma or equivalent with 18 post‑high‑school credits in behavioral health or related field.
• Two years of full‑time behavioral health work experience or four years total; degree preferred.
• Excellent written and oral communication; Apache language a plus.
• Valid Arizona driver’s license and reliable transportation.

Responsibilities

• Manage a caseload of clients who would benefit from behavioral health services.
• Coordinate transportation, outreach, and appointment setting for clients.
• Facilitate group counseling and skill training with ABHS clinicians.
• Document client contact, update assessments and treatment plans per AHCCCS/ABHS standards.
• Collaborate with schools, courts, tribal agencies, families, and partners.
• Provide trauma‑informed care and engage client support systems.
