Job summary

Children’s Crisis Treatment Center is seeking a Care Coordinator for its Blended Case Management (BCM) program. The role supports treatment planning and follows through with team recommendations for children aged 2–14 and their families presenting with crisis and behavioral health issues in a trauma-informed manner.

The coordinator will conduct interagency assessments, develop service plans, build family and community networks, and monitor services to ensure continuity of care across home,

Qualifications

• Bachelor’s degree in Social Work or related field is required.
• Experience engaging families and coordinating services across multiple systems is preferred.
• Ability to provide direct services to children and adolescents with emotional disturbances and their families.

Responsibilities

• Provide interagency assessment of child’s medical, family, social, and educational history.
• Develop service plans based on child and family strengths and needs.
• Build a network with family and community resources to support the child.
• Visit families in the community monthly or more to access resources identified in the plan.
• Link, coordinate and monitor services to keep the child stable at home, school and in the community.
• Follow through with psychological or psychiatric recommendations and monitor effectiveness.
