The Care Coordinator will support our Blended Case Management (BCM) program and follow through with treatment team recommendations in a faced paced environment and meet the needs of the children ages 2-14 and their families presenting with crisis and behavioral health issues in a trauma-informed manner.
A minimum of a Bachelor’s degree in Social Work, Social Science or Bachelor of Arts Degree from an accredited educational institution.
Ability to provide direct services to children and adolescents with serious emotional disturbances and their families.
Experience in community families with multiple problems.
