• Assists clients in identifying strengths, needs, and goals and develops an individualized case plan.
• Monitors mental health symptoms, functioning, and reality orientation with ongoing support.
• Provides skill-building in daily living, social and interpersonal relationships, and occupational capabilities.
• Coordinates referrals to internal and external programs/services to meet client needs.
• Ensures safety and security when working in homes, shelters, and community settings; travels as needed.
