Qualifications

• Professionals at this level have excellent communication skills with both client and community institutions.
• Bachelor's degree in a human service field is required.
• Three years in case management, vocational or skill development experience is preferred.
• Experience working with people who have serious and persistent mental illness preferred.

Responsibilities

• Complete comprehensive assessment of clients' mental health history, activities of daily living, and social history to form client-centered treatment plans.
• Assist clients in developing skills in daily living, social relationships, and occupational capabilities.
• Provide support and consultation to team members, client families, and community partners.
• Maintain all contact documentation and comply with agency standards.
• Utilize technology to deliver services and complete documentation in the electronic health record.
• Provide crisis intervention as needed and coordinate services with other community agencies.
