Job summary

Flourish Health in West Virginia seeks an In-Person CSED Therapist to provide trauma-informed individual and family therapy for children, adolescents, and young adults under the CSED Waiver. You will work in patients' homes and community settings across WV, building strong therapeutic relationships and coordinating with a multidisciplinary team.

Qualifications

• Master's degree in Social Work, Counseling, Psychology, MFT, or related field; independent practice license.
• Active West Virginia clinical license.
• Experience with CSED family therapy services.
• Experience treating under-served youth and families with complex needs.
• Knowledge of trauma-informed care and family systems approaches.
• Strong clinical judgment and documentation.
• Willingness to provide in-person home and community visits across WV.
• Credentialing/Medicaid compliance.
• Wraparound care models and PCSP familiarity.
• Caseload of 20-32 patients.
• Evening availability 2+ nights/week.
• Age 18+.
• Valid driver’s license and insurance.

Responsibilities

• In-person individual and family therapy at patients' homes and community settings.
• Follow Flourish’s trauma-informed treatment manual tailored to each patient.
• Support youth with significant emotional and behavioral challenges, including those at risk of higher levels of care.
• Develop and implement individualized therapeutic goals aligned with CSED Plan of Care.
• Help patients and caregivers develop coping, regulation, and communication skills.
• Build trusting relationships with patients and families.
• Involve caregivers as active treatment partners.
• Strengthen family resilience and system functioning.
• Promote autonomy and culturally responsive, person-centered care.
• Collaborate with multidisciplinary care team members and participate in treatment planning.
• Communicate changes in patient needs, progress, barriers, and safety concerns.
• Support continuity of care to prevent unnecessary hospitalization.
• Conduct ongoing clinical assessments and monitor progress toward goals.
• Identify safety concerns and work with crisis team.
• Maintain timely clinical documentation according to WV requirements.
• Engage in supervision, quality initiatives, and professional development.
• Deliver care per best practices and ethical WV standards.
