Job summary

Total Senior Care and Community Care of WNY is seeking a Scheduling and Transportation Coordinator in Allegany, NY. This full-time role focuses on planning and coordinating transportation services, scheduling medical appointments, and ensuring timely, compliant care plans for participants.

Responsibilities include coordinating with clinics and caregivers, supervising drivers, handling scheduling changes, and maintaining accurate records.

Qualifications

  • High School diploma or equivalent.
  • At least two years related work experience; supervisory experience preferred.
  • One-year experience working with the frail elderly.
  • One-year experience as a Transportation Coordinator preferred.
  • Acceptable driver’s license and automobile with current insurance.
  • Ability to work effectively in a team environment.
  • Demonstrated computer literacy.
  • Problem-solving and communication skills.
  • Must be able to move about freely, lift a minimum of 20 lbs., and have adequate hearing and vision.

  • Schedules medical appointments with coordination with clinic, home care and caregivers.
  • Serves as liaison to contracted transportation carriers dispatch offices.

  • Responds to grievances and resolves issues.
  • Receives and processes requests for service.
  • Assigns cases according to service requirements and notifies relevant parties of changes.
  • Maintains program records according to policy.
  • Participates in Interdisciplinary Team meetings and contributes to care plans.

Salary Range: $18.00 To $26.00 Hourly

SCHEDULING AND TRANSPORTATION COORDINATOR 35 hours per week; NO weekends or major holidays; Health insurance; PTO; 403b retirement;

BASIC PURPOSE AND SCOPE:

Under general supervision of the Scheduling and Transportation Supervisor, plans, organizes, and implements the transportation services of the day health center and medical services as well as scheduling medical appointments as established by the participant plan of care. Is responsible for the organization, implementation, and documentation of assignments for agency service personnel. Will ensure the attainment of agency scheduling/documentation goals in accordance with established policy and procedure, as well as supervision of service personnel in relation to these goals.

RESPONSIBILITIES:

Under limited supervision and in accordance with all applicable federal, state and local laws/

regulations and Total Senior Care and Community Care of WNY policies, procedures and guidelines,

this position:

  • Plans and coordinates all scheduling activities related to transportation for Total Senior Care participants.
  • Conducts initial and periodic transportation needs assessment to determine appropriate level of service necessary.
  • In accordance with participant plan of care and IDT authorization, schedules medical appointments with necessary coordination with clinic, home care and caregivers.
  • Serves as liaison to contracted transportation carriers dispatch offices.
  • Monitors for safety and ensures program compliance of daily activities of the contracted drivers/aides.
  • Responds to grievances and discrepancies and actively works to resolve issues.
  • Assists with scheduling of credentials, and inservice attendance.
  • Receives and processes requests for service.
  • Assigns cases in accordance with service requirements. Notifies personnel, patient, family, and/or other referring person/agency of scheduling changes.
  • Ensures that all necessary program records are maintained according to policy.
  • Participates as a member of the Interdisciplinary Team and contributes to the development and revision of the participant plan of care.
  • Updates care plan records for Home Care and Hospice. Exercises supervisory authority regarding scheduling problems and agency policy for personnel scheduled by way of corrective action initiation.
  • Maintains positive relationships with referral sources and the community through daily contacts.
  • Serves subject-to-call on a rotating basis.
  • Takes and documents referral information when received. Obtains needed referral information from referral source and or community physicians.
  • Responsible for training new Aides on CellTrak and for resolving any issues that may arise.
  • Completes other reasonable duties as assigned.
  • Adheres to and reflects organizational values in daily work.
  • Serves on agency committees as may be assigned.
  • Maintains an obligation to report wrongdoing/violation of agency policies, applicable federal, state, and local laws, and rules and regulations pertaining to agency operations, to immediate supervisor or identified compliance officer.
  • Completes all mandatory inservice education programs and completes any other additional inservice hours that are minimally required for the position.

MINIMUM REQUIREMENTS:
  • High School graduate or equivalent.
  • At least two years related work experience; supervisory experience preferred.

  • One-year experience as a Transportation Coordinator preferred.
  • Acceptable driver’s license and automobile available for use with current insurance.
  • Ability to work effectively in a team environment.
  • Ability to effectively handle difficult and unusual interpersonal situations.
  • Demonstrated computer literacy.
  • Problem-solving and communication skills.
  • Must be able to move about freely, lift a minimum of 20 lbs., possess manual dexterity and have visual acuity to see. Must be able to hear and verbally communicate.
