Job summary

Responsibilities

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulate and implement a plan of care including discharge planning; collaborate with patient/family and health team.
• Coordinate and supervise nursing care, ensuring communication with caregivers and continuation of care.
• Evaluate patient progress and modify the care plan as needed; document assessments and interventions in the patient record.
