Job summary

Riverstreet Manor in Wilkes-Barre, PA is seeking a Transportation CNA (PRN/Per Diem) to safely transport residents to dialysis and assist with non-clinical support across the facility. You will coordinate with Recreation staff for activities and with nursing for appointments, ensuring safety, dignity and timely arrival.

The role requires PA CNA license, high school diploma, dependability, strong communication, and prior LTC/healthcare exposure is preferred.

Qualifications

• Active CNA license in PA.
• High school diploma or GED.

• Excellent phone and communication skills.
• Experience in healthcare, LTC, transportation or hospitality preferred.
• Ability to assist with mobility and wheelchairs and complete orientation/training.

Responsibilities

• Safely transport residents to and from the dialysis unit and assist with comfort and non-clinical tasks.
• Coordinate with Recreation Director and staff for transporting residents to activities.
• Coordinate with medical and nursing personnel for appointments.
• Ensure emergency equipment is available and in good condition.
• Assist in moving equipment and ensuring vehicles are ready for tasks.
• Assist residents entering and exiting the facility vehicle.

Position Summary

The Transport Aide is primarily responsible for safely transporting residents to and from the facility's on-site dialysis unit and assisting with resident comfort, hydration, and basic non-clinical support services throughout the facility. This position plays a key role in ensuring residents arrive at appointments safely, timely, and with dignity while supporting the nursing team with additional duties as assigned.

Duties and Responsibilities of the Transportation CNA:

• Coordinate with Recreation Director and Recreation Assistantregardingtransporting residents to scheduled activities.

  • Coordinate with Facility medical and nursing personnel regarding transporting residents to scheduled medical appointments.

• Use protective clothing or devices when handling infectious waste and/or blood/body fluids.

• Ensure that emergency equipment (i.e. triangles, fire extinguisher, and first-aid kit) are available and in good working condition.

• Use supplies in an efficient manner to avoid waste.

• Assist in the moving and arranging of equipment and supplies to assure that the Facility’s vehicle are ready for scheduled appointments and activities.

Requirements of the Transportation CNA:

• Must have an active CNA license in PA

• Must have a high school diploma/GED

• Must be dependable, timely, friendly, good phone skills, and have excellent customer service.

• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

• Previous long-term care, healthcare, transportation, or hospitality experience preferred.

• Ability to push wheelchairs, transport residents safely, and perform frequent walking, bending, lifting, and standing.

• Must successfully complete facility orientation and required training.

• Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen

Physical Requirements

• Frequent walking, standing, pushing, pulling, bending, and reaching.

• Ability to transport residents in wheelchairs and assist with mobility activities.

• Ability to lift and move up to 50 pounds with assistance when appropriate.

Working Conditions

• Skilled nursing and rehabilitation environment.

• Exposure to residents with varying physical and cognitive needs.

• May work weekends, holidays, and flexible schedules as needed.
