AdventHealth seeks a PRN Social Worker to perform comprehensive psychosocial assessments for transplant patients and donors in Denver. You will collaborate with the transplant team to create individualized care plans and provide ongoing counseling and discharge planning.
Candidates must hold an MSW, an LCSW, and have 1+ year of related clinical social work experience. This role focuses on holistic patient care within a faith-based health system in Colorado.

Conducts comprehensive assessments of patient and donor psychosocial, cultural, and spiritual needs through interviews, observations, and clinical data.
Evaluates family relationships, support systems, mental health history, personality traits, and coping skills to inform care plans.
Collaborates with the transplant team to develop and implement individualized care plans that address the holistic needs of patients and donors.
Provides counseling and support to patients and donors, facilitating decision-making and addressing concerns related to the transplant process.
Participates in multidisciplinary rounds and transplant team meetings to discuss patient and donor progress and care plans.
Refers patients and donors for additional evaluations, such as neuropsychological assessments, as needed.
Engages in discharge planning, coordinating healthcare and community services to support patients and donors post-transplant.
Educates patients and donors on the financial aspects of transplantation, including insurance coverage and available resources.
Advocates for patients and donors, ensuring their needs and preferences are respected and addressed.
Provides individual or group therapy sessions to support patients and donors in coping with the transplant process.
Other duties as assigned.

Pay Range: $62,108.80 - $115,544.00
