Job summary

AdventHealth Denver is seeking a transplant social worker for PRN day shifts. You will conduct comprehensive psychosocial assessments of patients and donors, collaborate with the transplant team to craft holistic care plans, and provide counseling to support decision-making throughout the transplant process.

Qualifications

• Master's degree in Social Work required.
• LCSW licensure required for practice.
• Minimum 1+ year related clinical social work experience.

Responsibilities

• Assess psychosocial, cultural, and spiritual needs of patients and donors.
• Collaborate with transplant team to develop individualized care plans.
• Provide counseling to patients and donors to support decision-making.
• Participate in rounds and multidisciplinary meetings to review progress.

Job Description:

Conducts comprehensive assessments of patient and donor psychosocial, cultural, and spiritual needs through interviews, observations, and clinical data. Evaluates family relationships, support systems, mental health history, personality traits, and coping skills to inform care plans. Collaborates with the transplant team to develop and implement individualized care plans that address the holistic needs of patients and donors. Provides counseling and support to patients and donors, facilitating decision-making and addressing concerns related to the transplant process. Participates in multidisciplinary rounds and transplant team meetings to discuss patient and donor progress and care plans. Refers patients and donors for additional evaluations, such as neuropsychological assessments, as needed. Engages in discharge planning, coordinating healthcare and community services to support patients and donors post-transplant. Educates patients and donors on the financial aspects of transplantation, including insurance coverage and available resources. Advocates for patients and donors, ensuring their needs and preferences are respected and addressed. Provides individual or group therapy sessions to support patients and donors in coping with the transplant process. Other duties as assigned.
