Qualifications

• Graduated from an ACPE-accredited school of pharmacy or higher.
• Completion of a pharmacy practice residency or 2 years of clinical experience.
• Active pharmacist license in Georgia or eligible to obtain upon hire.

Responsibilities

• Provide direct patient care and medication therapy management.
• Participate in multidisciplinary rounds and clinical decision-making.
• Monitor and adjust immunosuppression and other medications.
• Conduct medication reconciliation and assess drug interactions.
• Develop and monitor individualized pharmacotherapy plans.
• Provide education to patients, caregivers, and staff.
• Contribute to quality improvement and regulatory compliance.
• Participate in clinical research and data analysis.
