Piedmont Healthcare Inc. is seeking an Abdominal Transplant Clinical Pharmacist to provide specialized pharmaceutical care for patients undergoing kidney, pancreas, and liver transplantation. This role involves working closely with a multidisciplinary team to manage medications effectively and improve patient outcomes.

The ideal candidate will have a Bachelor’s Degree in Pharmacy and significant clinical experience, along with the opportunity to work from home one to two days a week. Responsibilities include direct patient care, medication management, and contributions to protocol development.

Qualifications

• ACPE-accredited School of Pharmacy graduate required.
• Completion of pharmacy practice residency or two years of clinical experience.
• Active pharmacist license in Georgia or eligible to obtain upon hire.

Responsibilities

• Provide direct patient care and medication therapy management.
• Participate in multidisciplinary inpatient rounds.
• Conduct medication reconciliation and assess for adherence issues.
• Develop individualized pharmacotherapy plans.
