Job summary

The University of Illinois Hospital & Clinics (UI Health) seeks a full-time Advanced Practice Provider (APP) for the Transplant Outpatient program. The APP will join a multidisciplinary team to deliver high-quality outpatient transplant care across all ages, under physician supervision.

The role includes performing histories and exams, developing care plans, prescribing within privileges, teaching patients and families, and contributing to quality improvement and potential research activities.

Qualifications

• Graduation from a nationally accredited APRN or PA training program.
• Current certification appropriate to APP category (ACNP-BC/ AGACNP-BC or PA-C).
• Current Illinois license as APRN-CNP or PA-C.

Responsibilities

• Works collaboratively with care teams to provide high-quality outpatient transplant care.
• Performs history and physical exams and develops patient care plans.
• Prescribes medications and therapies within clinical privileges and scope.
• Contributes to teaching patients, families, and students; supports research and evidence-based practice.
• Leads quality-improvement initiatives and participates in protocol development.
